CVS Health is the nation’s leading health solutions company, dedicated to transforming health care. The Bilingual Analyst Case Management position involves assessing and coordinating care for members in a field-based role, requiring travel to meet with members face to face.
Responsibilities
- Assessing, planning, implementing, and coordinating all case management activities with members to evaluate the medical needs of the member to facilitate the member's overall wellness
- Effectively manage a caseload that includes supportive and medically complex members
- Develop a proactive course of action to address issues presented to enhance the short and long-term outcomes as well as opportunities to enhance a member's overall wellness through integration
- Determine appropriate services and supports due to member's health needs; including but not limited to: Prior Authorizations, Coordination with PCP and skilled providers, Condition management information, Medication review, Community resources and supports
Skills
- Must live in Tampa, FL (Specifically in zip code 33614 or within a 2-3 mile radius)
- Bi-lingual (English and Spanish)
- Willing and able to travel 50-75% of their time to meet members face to face in Hillsborough County and surrounding areas in FL
- 1+ years of experience in behavioral health or a long-term care setting
- Computer proficiency in Microsoft Word, Excel, and Outlook
- Bachelor's Degree required, preferably in Social Work or related field (social services field - psychology, health administration)
- Ability to multitask, prioritize and effectively adapt to a fast-paced changing environment
- Effective communication skills, both verbal and written
- Managed care experience
- Case management and discharge planning experience
