Job Summary:
We're seeking a highly motivated and compassionate individual to join our team as a Community Care Worker. In this role, you'll work with clients to achieve program goals, utilizing our proprietary platform and data management system. Your responsibilities will include:
- Providing population health services to assigned clients on an ongoing and emergency basis
- Developing and maintaining accurate knowledge of program requirements, tools, and community resources
- Organizing and documenting daily schedules, adhering to safety protocols, and ensuring compliance with HIPAA and confidentiality policies
- Demonstrating competency by meeting or exceeding minimum caseload requirements and department work performance expectations
- Communicating effectively with supervisors, customers, and co-workers to build rapport and resolve issues
Requirements:
To succeed in this role, you'll need:
- An Associate's Degree in social work or a related field, or a High School Diploma/GED with 4 years of related experience and/or training in healthcare and/or customer service
- 2 years of experience and/or additional education in social work or a related field, with knowledge of medical terminology and/or experience in a customer service position (preferably in a healthcare setting)
- A valid driver's license, current automobile insurance, and availability to work evening hours (5:00 pm - 8:00 pm) and some weekends
