Licensed Vocational Nurse (LVN) Job Description: The Licensed Vocational Nurse (LVN) provides skilled nursing services to patients in the home health setting under the direction and supervision of a Registered Nurse and in accordance with the physician-approved plan of care. The LVN supports the coordination and delivery of quality patient care by performing skilled clinical interventions, monitoring patient status, educating patients and caregivers, and communicating changes in condition to the appropriate members of the clinical team. Responsibilities include: Providing skilled nursing care in accordance with the established plan of care and within the LVN scope of practice Monitoring and documenting patient condition, including vital signs, pain level, skin integrity, cardiopulmonary status, safety concerns, and response to care Performing skilled interventions such as wound care, dressing changes, medication teaching, disease process education, Foley catheter care, ostomy care, diabetic care, and other ordered nursing treatments as appropriate Observing, identifying, and reporting changes in the patient’s condition to the RN Case Manager, physician, and interdisciplinary team as needed Educating patients and caregivers regarding medication management, disease management, infection prevention, fall prevention, safety precautions, and proper care techniques Collaborating with the RN Case Manager and other members of the interdisciplinary team to support continuity of care and patient-centered outcomes Maintaining accurate, timely, and complete clinical documentation in accordance with agency policies, state regulations, Medicare requirements, and professional standards Following infection control protocols, patient safety standards, and all applicable home health regulatory requirements Qualifications: Current and active California LVN license Minimum of 1 year of nursing experience preferred; home health experience strongly preferred Current CPR/BLS certification Valid driver’s license, reliable transportation, and proof of auto insurance Ability to provide care independently in the home setting while maintaining communication with the supervising RN and clinical team Strong assessment, communication, documentation, and time management skills Professional, dependable, compassionate, and patient-centered approach to care
