Primary Care Physician
Locations: Dartmouth, Leominster or Springfield, MA
Compensation: $275,000–$300,000 annually, based on experience

Key Responsibilities
• Provide primary, preventive, urgent, and chronic care to PACE participants
• Conduct initial and biannual assessments and contribute to individualized care plans
• Participate actively in interdisciplinary team (IDT) meetings
• Coordinate specialty care and transitional care services
• Deliver care across settings including clinic, home, and inpatient facilities
• Participate in limited after-hours call rotation
• Maintain timely and accurate clinical documentation
Qualifications
• MD or DO licensed in Massachusetts; DEA required
• Board-certified or board-eligible in Internal Medicine, Family Medicine, or Geriatrics (geriatrics preferred)
